Client Portal
Current clients may manage appointments, access invoices, send messages, or join telehealth sessions via the Client Portal. If you do not yet have a portal account, please email admin@westhoustoncounseling.com to request an account.
Adult Intake Form
All new clients need to complete an intake form prior to attending their first session. If the session is for couples or family therapy, each person attending the session should fill out a separate intake form. Please bring your ID and insurance card (if applicable) to the first session.
Minor Intake Form
Parents or guardians should provide an intake form for their minor prior to attending the first session. Please provide a separate intake form for each client attending. Please bring an ID, insurance card, and court paperwork (if applicable).
